• Statement Regarding a Lost or Stolen Passport DS-64 (1 copy completed but not signed)
• Proof of Child's US Citizenship
• Evidence of Child's Relationship to Parents/Guardians
• Parent's proof of identity
• Parental Statement of Consent DS-3053 if parent cannot accompany child in Step 2.
• Two passport photos
• Letter of Authorization
• Proof of departure
• Check to “US Department of State” in the amount of $127

Print list of acceptable documents.

Bring documents to official acceptance agent near you. http://iafdb.travel.state.gov

The acceptance agent will seal an evelope containing your

• application
• documents
• check for $127 payable to US Department of State

Pay Acceptance Agent fee of $30.00.

Take the envelope with you.

DO NOT LEAVE THE ENVELOPE with the acceptance agent.

DO NOT OPEN THE ENVELOPE after you leave the acceptance agent!

Minors Under Age 14:
Each minor child shall appear in person.
Applications for children under 14 require both parents' or legal guardians' consent.

Both parents must accompany child to acceptance agent or submit Form DS-3053
